Admissions to Foundations for Living

Located in Mansfield, Ohio, Foundations for Living is a secure, residential treatment center for adolescents ages 10 through 17, with a minimum IQ of 65. Program track includes a long-term treatment program.

Long-Term Treatment Admission Criteria:

  • Ages 10–17 in need of intensive mental health and/or substance-related disorder treatment
  • Minimum IQ of 65
  • No current treatment needs for sex offenses
  • School eligible (no high school graduates/GED)
